Sherborne PCN Frailty Nurse

Working alongside our established Frailty Nurse, the PCN Clinical Director and GP Lead, to support the PCN with the continued development of our frailty service. Planning and providing anticipatory and current care for identified patients from one or two practices in the PCN. Including treatment, preventative care, screening, future planning, and patient education. Usually in the patients home, in the practice, in one of our local care homes, or whilst working at an outreach or PCN event To act as a focal point of communication between patients and members of our primary health care team, Dorset Healthcare, and other services in the community. Establish a collaborative working relationship with the Care Homes allocated to the PCN, colleagues within Secondary Care, and other aligned services. Manage own case load, working independently and as part of the PCN wide team, continuing to deliver excellent healthcare to our 25,000 patients.
