Social Prescribing Link Worker

Job SummaryDerbyshire Dales PCN is seeking a proactive, community-minded Social Prescriber to deliver targeted, data-driven support as part of our innovative centralised model of care. Based around monthly themed initiatives, youll work independently and collaboratively to reduce health inequalities by connecting patients to non-clinical support across our communities. This role involves both 1:1 work and facilitation of tailored group sessions for specific cohorts such as isolated older adults, frequent attenders, or those at risk of escalation. Occasional weekend work will be expected to support community outreach and group activities. Key Responsibilities- Thematic Interventions: Lead proactive, population-based initiatives based on monthly priorities, informed by population health data and practice insights. - Group Facilitation: Develop and deliver structured group sessions focused on improving wellbeing and resilience among identified patient cohorts. - Caseload Management: Work independently to assess, support, and monitor patients referred through PHM tools, practice teams, or direct outreach. - Collaborative Working: Build strong links with local GPs, community services, and voluntary sector organisations to strengthen referral pathways and support. - Weekend Outreach: Participate in weekend community events, engagement initiatives, or group activities as required. - Data Use and Recording: Use clinical systems (e.g., EMIS, SystmOne) and PHM tools to record activity, track outcomes, and support service evaluation. - Supervision and Support: Engage in monthly clinical supervision and peer learning opportunities, sharing best practices with local and regional networks. Professional Development RequirementsTo ensure high standards of delivery and professional growth, the successful applicant: - Has completed the NHS England online learning programme via the Personalised Care Institute website. - Attends peer support networks organised at place or system level by the regional Integrated Care System (ICS). - Is enrolled in, undertaking, or qualified from appropriate training to achieve a Level 3 occupational standard (as outlined in the Workforce Development Framework for Social Prescribing Link Workers) if not already holding a Level 3 qualification. - For those who already hold a Level 3 qualification, further training is recommended but not mandatory.
