Job description

Working within a multidisciplinary team, responsibilities include completing assessments, developing and reviewing care plans, supporting discharge planning, managing risk, securing appropriate care funding and maintaining accurate records. The post holder must organise and prioritise their workload, work across a variety of settings, and contribute to service improvement, audits and evidence-based practice.They are expected to maintain professional development, participate in supervision, support staff and students, use resources responsibly, and practise autonomously within professional standards while managing a defined caseload with appropriate support.
