Job description

MAIN TASKS AND RESPONSIBILITIES • Responsible to present a welcoming introduction to the centre and units for patients and visitors • Assisting in the day-to-day administration of the centre: e.g. arranging transport, booking appointments, answering the centre and unit telephones, maintaining clinical systems • Ensuring all relevant documentation for the admission, discharge and death of patients from the unit is in place including requesting patient records, maintaining patient lists, and the correct archiving following policy • Liaising with social services and key community staff to ensure all necessary support services are requested prior to discharge.
