Job Description Job TitleAdvanced Clinical Practitioner (ACP) Reports toPractice Manager and GP Partners Accountable toGP Partners Job PurposeThe Advanced Clinical Practitioner (ACP) will provide autonomous, high-quality, patient-centred clinical care within Caritas Group Practice. Working within their scope of practice, the ACP will assess, diagnose, treat and manage patients presenting with undifferentiated and diagnosed conditions, using advanced clinical reasoning and evidence-based practice. The ACP will work collaboratively with GPs and the wider multidisciplinary team to improve access to primary care, deliver safe and effective clinical services, support quality improvement initiatives and contribute to the ongoing development of the practice. Main Duties and ResponsibilitiesClinical Responsibilities • Assess, examine, investigate, diagnose and manage patients presenting with acute, urgent and long-term conditions. • Independently prescribe medications in accordance with current legislation and best practice (where qualified). • Order, interpret and act upon diagnostic investigations including pathology and imaging. • Formulate appropriate management plans and provide follow-up where required. • Refer patients appropriately to secondary care and community services. • Provide clinical advice, health education and health promotion to patients and carers. • Undertake home visits where appropriate. • Recognise and manage clinical risk, safeguarding concerns and medical emergencies. • Maintain accurate, timely and contemporaneous clinical records using SystmOne. • Work within national and local clinical guidelines and practice protocols. Service Delivery • Manage a defined clinical workload, including same-day, urgent and routine appointments. • Support effective patient flow and improved access to services. • Work collaboratively with GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, First Contact Physiotherapists, Mental Health Practitioners, Social Prescribers and other members of the multidisciplinary team. • Participate in multidisciplinary team meetings where appropriate. • Contribute to service redesign and the implementation of new models of care. Clinical Governance • Maintain high standards of clinical practice and professional conduct. • Participate in clinical audit, quality improvement and service evaluation. • Contribute to significant event reviews and clinical governance activities. • Support the implementation of national guidance and local policies. • Ensure compliance with infection prevention and control procedures. • Maintain patient confidentiality and comply with GDPR and information governance requirements. Leadership and Professional Responsibilities • Demonstrate leadership within the clinical team. • Act as a role model for evidence-based practice. • Support and mentor junior colleagues, students and newly appointed staff where appropriate. • Participate in appraisal, mandatory training and continuing professional development. • Maintain registration with the appropriate professional body and comply with revalidation requirements. • Keep up to date with developments in advanced clinical practice and primary care. Communication • Communicate effectively with patients, relatives, carers and colleagues. • Work collaboratively with external healthcare professionals and partner organisations. • Manage sensitive conversations with professionalism, empathy and confidentiality. Health and Safety • Comply with all practice Health and Safety policies. • Report incidents, accidents and near misses in accordance with practice procedures. • Maintain a safe working environment for patients, visitors and colleagues.
