The WEoLCCS provides a dedicated, specialised care coordination service and central point of contact for patients likely to be in their last year of life and those who care for them(formally and informally). The main responsibilities of the Lead Nurse are: • To act as a community nursing end of life champion and inspire others demonstrating a range of skills • To directly manage and lead the Services Health & Personal Care Delivery Team • To provide clinical support to members of the Service • To work closely with the Services care delivery team, Continuing Health Care, and the care agencies to ensure care is planned to meet the unique and changing needs of the patients, their families and friends • To build and maintain professional relationships (internally and externally) and to promote Trinitys services • To work flexibly within the Clinical Hub Services team when there is a need Job description Job responsibilities The WEoLCCS provides a dedicated, specialised care coordination service and central point of contact for patients likely to be in their last year of life and those who care for them(formally and informally).
