• Coordinate referrals across multiple services • Receive, review and triage referrals into the CYP programme • Build collaborative relationships across organisations • Provide advocacy, tailored support and empower self-management within sessions • Coordinate onward referrals to the most appropriate services • Support children, young people and families to navigate local services • Develop individual support plans where appropriate • Maintain regular contact with families whilst awaiting support • Escalate safeguarding or clinical concerns appropriately • Maintain accurate clinical records using digital systems • Represent the CYP Programme across Hillingdon Partnership Working • Develop strong working relationships with: • GP Practices • Primary Care Networks • CAMHS • CYP Mental Health SPA • MHST • School Nursing • Family Hubs • Family Help Teams • Local Authority Services • Third Sector organisations • Community providers • Act as the first point of contact for advice regarding available CYP services.
