Paramedic, Advanced Nurse or Clinical Practitioner

The post holder will work within their professional scope of practice to provide high-quality clinical care for patients registered with East Norfolk Medical Practice. They will work closely with GPs and the wider multidisciplinary team to assess, diagnose, treat and manage a wide variety of patient presentations, supporting both routine primary care and urgent care services. Clinical Responsibilities • Assess, diagnose and manage patients presenting with acute and chronic conditions. • Utilise advanced clinical assessment and decision-making skills. • Conduct face-to-face and telephone consultations. • Request and interpret investigations including blood tests, imaging and ECGs. • Develop, initiate and review patient management plans. • Review and action clinical results, referrals and correspondence. • Undertake home visits and care home visits as required. • Refer patients appropriately to primary, community and secondary care services. • Work within local and national guidelines and clinical protocols. • Recognise limitations of own knowledge and seek support when appropriate. Team Working • Work collaboratively with GPs and the wider multidisciplinary team. • Contribute to high standards of patient care and service delivery. • Participate in multidisciplinary discussions and service development initiatives. • Support continuity of care for patients. Education and Development • Participate in regular educational meetings and clinical supervision. • Maintain professional registration and continuing professional development. • Support students and junior colleagues where appropriate. • Reflect on practice and participate in learning opportunities to enhance clinical knowledge and skills. Leadership • Demonstrate professional leadership within the multidisciplinary team. • Act as a role model for clinical excellence and professionalism. • Promote effective communication and collaborative working. The post holder may be required to travel between Practice sites and undertake visits to patients in their own homes, care homes or temporary residences.
