Recovery Navigator (Access Community Mental Health Team)

Collaborate with health professionals, VCSE partners, and other support networks to provide seamless experience. Conduct support conversations using tools like DIALOG+ to create tailored, strengths-based support plans. Deliver person-centered, culturally sensitive, and trauma-informed interventions. Use your own lived experience (if applicable) to provide hope and connection. Support individuals with navigation around housing, employment, relationships, and mental wellbeing.
